Whānau Tahi Dynamics 365 Dashboards

These dashboards are available as part of Whanau Tahi Intake, Whanau Tahi Back Office and/or Whanau Ora Planning Model-Driven App.

1. Demographics Dashboard

Available in Whanau Tahi Intake, Whanau Tahi Back Office and Whanau Ora Planning Model-Driven App.
Intended purpose – Breaks down clients by ethnicity, age group and suburb. In addition, there’s a chart that summarizes Maori clients by tribe.
Intended users – All kaimahi

2. Group Program

Available in Whanau Tahi Intake, Whanau Tahi Back Office and Whanau Ora Planning Model-Driven App.
Intended purpose – Displays group programs that have been created in the last 12 months and also their related sessions. There is also a list that displays the sessions related to the group programs.
Intended users – All kaimahi

3. Navigator Dashboard

Available in Whanau Tahi Intake, Whanau Tahi Back Office and Whanau Ora Planning Model-Driven App.
Intended purpose – Displays inbound referrals, activities, overdue outcomes and a list of plans for the current user.
Intended users – All kaimahi

4. Navigator Manager Dashboard

Available in Whanau Tahi Intake, Whanau Tahi Back Office and Whanau Ora Planning Model-Driven App.
Intended purpose – Displays all plans, outcomes and referrals by owner, providing the Manager with a holistic view.

5. Referrals Management Dashboard

Available in Whanau Tahi Intake, Whanau Tahi Back Office and Whanau Ora Planning Model-Driven App.
Intended purpose – Summarizes referrals by service, status and owner. There is also a list displays all the active referrals.

6. Referrals, Plans and Outcomes

Available in Whanau Tahi Intake, Whanau Tahi Back Office and Whanau Ora Planning Model-Driven App.
Intended purpose – Displays active referrals, plans and outcomes for the current user.
Intended users – All kaimahi

7. Tamariki Ora Kaimahi Dashboard

Available in Whanau Tahi Intake, Whanau Tahi Back Office and Whanau Ora Planning Model-Driven App.
Intended purpose – Summarizes assessment activities by different metrics. There is also a chart that summarizes plan activity by owner.

Intended purpose – Summarizes assessment activities by different metrics. There is also a chart that summarizes plan activity by owner.

9. Team Leader Dashboard

Available in Whanau Tahi Intake, Whanau Tahi Back Office and Whanau Ora Planning Model-Driven App.
Intended use – Provides an overview of plans created by month, referrals, outcomes, goals and activities. It also displays plans, outcomes and activities by owner.

10. Contract Management Dashboard

Available in Whanau Tahi Back Office Model-Driven App only.
Intended use - Displays incoming referrals by service.
Intended users - All kaimahi

11. Contract Performance Dashboard

Available in Whanau Tahi Back Office Model-Driven App only.
Intended use - This chart is dependent on goals and metrics set by the admin for the different services. It displays targets for the diffent services and current progress till date.

12. Referral Triage Dashboard

Available in Whanau Tahi Back Office and Whanau Ora Planning Model-Driven App only.
Intended use - Displays incoming referrals that are in triage, by proprity and reason. There is also a list that dislays the referral items.

13. Organization Insights Dashboard

Available in Whanau Tahi Back Office Model-Driven App only.
Intended use - Displays the default organization insights dashboard as configured by Microsoft.

Intended use - Displays a list of activities created by students.
