ADOM Form Updates

  • Made the Responsible Service Team and Responsible HCP fields mandatory.
  • Updated Business Rules to hide questions 7b, 7d and 7f depending on answers given.

Care Team Roles

The "Plan Access" tab has been renamed to "Care Team", and the ability to add specific Care Team Roles under the Care Team Tab has been added.

Care Team Roles offer the following:

  • Ability to specify specific Staff or Clinician's roles in the context of a Service, or an Individual and Service, or Plan
  • Structured and non-structured role specifications
  • History of both Inactive and Active Care Team Role participants

Care Team Roles operate in addition to, and in isolation from, Plan Access records e.g., a person can be recorded to have a Care Team Role, but not necessarily be given Plan Access.

Quick Create on Comprehensive Assessment Form

Quick create functionality added to Comprehensive Assessment forms. This allows for the creation of ADOM, Arotake Ahorangi, Mental State Exam, Risk Assessment and My Plan quickly from an existing Comprehensive Assessment record.

Wellness and Safety Formulation Form Updates

  • Review and Action Plan Tab now allows for the adding of multiple Review and Action Plans for the Assessment.
  • Tooltips removed from all fields under 'Formulation' tab.

Comprehensive Assessment Form Updates

  • Additional information added to Referral Details section.
  • Added 'Created On' and 'Created By' fields to all Child Assessments appearing under Comprehensive Assessment.

  • DOB to be included in the header
  • Typo: 'Martial' status corrected 'Marital' status
  • Repositioning of several sections

Minor Adjustments

Adjustments to ADOM Form

Corrected the position of numerous fields on the form.

A tooltip has been added to 'Privacy Indicator' field under Basic Details tab.

Adjustments to Clinical Form PDF Export Templates

Adjustments were made to the IDP Review PDF Template, MHAS Discharge Summary PDF Template, and Wellbeing Recovery Plan PDF Template.

Adjustments to My Plan Form

  • Removed Kaupapa Tab and added kaupapa description for the My Plan at the top of 'Wellbeing/Advance Care Plan' tab.
  • Adjusted several controls to be left-justified and make the layout consistent.
