Referral Form Updates

New fields have been added to the Referral Form. Notice tab

PrimHD Information tab

  • Mental Health Triage Scale

A business rule has been created that has the Referral To and the Referral End on the PrimHD Information tab read only if the referral is NOT enrolled. If the referral is enrolled (e.g. Referral Status = Active), then the fields are not read only.

Formulation Tab (New)

Formulation Response Plan Outcome section (New)

Individual Card on Plan

The Plan Overview page shows an Individual card on click of any node associated with a Individual record. The Individual card contains Legal Status, Latest HoNos score, Leave Status and NHI number.

The Individual can be opened from the card, using a new "Open Individual" button.

Activity Header Changes - NHI and Legal Status

The following activities now have the NHI and Legal Status at the in the header. They are populated by the NHI and current legal status (at the time):

  • Alert
  • Appointment
  • Assessments
  • Case Note
  • Consent

  • Email
  • Exit
  • Goal
  • Group Activity
  • Phone Call
  • Referral (Outbound)

Individual Backend Data Changes

New columns have been added to Individual Table, this data is copied across by a system automation process.

The following information is now available for an Individual:

  • Current Legal Status
  • Current Leave Status
  • Current HoNOS Score
  • Current HoNOS Score Date
  • Current HoNOS Score Type

An 'Attendee/s' free-text field has been added to capture meeting attendees.

Hovering over 'Review Date' field now shows the help text 'Due Date'.

'Consent Type' dropdown also features the following choices now:

  • Referral
  • Assessment
  • Discharge
  • Whanau Involved

Mental Health Clinical Forms

Three mental health clinical forms have been added:

  • Mental State Examination
  • My Plan (Shared care planning)
  • Risk Assessment

Unified Comprehensive Clinical Form Implementation

Comprehensive Clinical form now features

  • Individual Details
  • Individual Legal Status Details
  • ADOM Clinical form
  • Mental State Clinical form
  • My Plan (Shared care planning) Clinical form
  • Risk Assessment (Wellness and Safety Formulation) Clinical form
  • Arotake Ahorangi Clinical Form

A system automated process also fetches the latest completed clinical forms of each sort when a user opens the Comprehensive clinical form.

Form Printing

For clinical users, the following reports can be printed:

  • My Plan assessment
  • Risk Assessment (Wellness and Safety Formulation) Clinical form

These are available for printing via Run Report drop down menu.

Core Documents Visibility

For clinical users, the Core Document tab hides if System Config record Connected_Care_Connection has a value of 'false' or does not exist.

Plan Report Updates ("Run Report" drop down)

Add a Individual dropdown on all Reports that can be run from the Plan Form (using the "Run Report" button in the ribbon), so that a user can run the report filtered to selected Individual(s).

Clinical Dashboard (User Homepage)

The Clinical Dashboard (a newly created Dashboard, which can be leveraged or modified) is now the landing page for the Whānau Ora Planning apps (Mai i Kōwhai, Ara Hinengaro).

Supplementary Consumer Record Form

The main form of Supplementary Consumer Record now features questions and choices from Ministry of Health guidelines.
